Request for Applications: Healthy Horizons Regional Hub Organizations 

The New Mexico Health Care Authority (HCA) is issuing this Request for Applications (RFA) to solicit applications from eligible organizations to serve as Regional Hub Organizations for Healthy Horizons, a core initiative under New Mexico’s Rural Health Transformation (RHT) Program.

Applications must be submitted electronically through Submittable by the deadline identified in Section II.A. A submission that is not fully complete and submitted by the deadline may be deemed late or incomplete and may be excluded from review.

Submission Instructions 

  • All proposals must be submitted electronically via Submittable by the deadline.
  • The system will time-stamp submissions and automatically close at the exact deadline -- please allow sufficient time for uploads. 
  • Proposals submitted via fax, email, or other means outside of Submittable will not be accepted. 
  • Incomplete or late submissions (including those delayed by antivirus/security software or file upload errors) will be deemed non-compliant

Request for Proposals: Rural Health Innovation Fund (RHIF) 

The New Mexico Health Care Authority (HCA) is issuing this Request for Proposals (RFP) to solicit competitive, sealed proposals from qualified Offerors to implement Rural Health Innovation Fund (RHIF) projects in support of New Mexico’s Rural Health Transformation (RHT) Program. 

RHIF is intended to support implementation-focused health care innovation projects that serve New Mexico communities. Eligible Offerors may include health care providers, Tribal entities, local public health entities, community-based organizations, local governments, academic institutions, regional partnerships, vendors, technology partners, equipment suppliers, and other organizations or entities that can demonstrate a direct role in implementing the proposed project and a clear connection to New Mexico’s rural, frontier, or Tribal communities.

Applications must be submitted electronically through Submittable by the deadline identified. A submission that is not fully complete and submitted by the deadline may be deemed late or incomplete and may be excluded from review.
